STACKED REGRESSION WITH A GENERALIZATION OF THE MOORE-PENROSE PSEUDOINVERSE
In practice, it often happens that there are a number of classification methods. We are not able to clearly determine which method is optimal. We propose a combined method that allows us to consolidate information from multiple sources in a better classifier. Stacked regression (SR) is a method for forming linear combinations of different classifiers to give improved classification accuracy. The Moore-Penrose (MP) pseudoinverse is a general way to find the solution to a system of linear equations. This paper presents the use of a generalization of the MP pseudoinverse of a matrix in SR. However, for data sets with a greater number of features our exact method is computationally too slow to achieve good results: we propose a genetic approach to solve the problem. Experimental results on various real data sets demonstrate that the improvements are efficient and that this approach outperforms the classical SR method, providing a significant reduction in the mean classification error rate
Evolutionarily Tuned Generalized Pseudo-Inverse in Linear Discriminant Analysis
Linear Discriminant Analysis (LDA) and the related Fisher's linear discriminant are very important techniques used for classification and for dimensionality reduction. A certain complication occurs in applying these methods to real data. We have to estimate the class means and common covariance matrix, which are not known. A problem arises if the number of features exceeds the number of observations. In this case the estimate of the covariance matrix does not have full rank, and so cannot be inverted. There are a number of ways to deal with this problem. In our previous paper, we proposed improving LDA in this area, and we presented a new approach which uses a generalization of the Moore-Penrose (MP) pseudo-inverse to remove this weakness. However, for data sets with a larger number of features, our method was computationally too slow to achieve good results. Now we propose a model selection method with a genetic algorithm to solve this problem. Experimental results on different data sets demonstrate that the improvement is efficient
Doping in Sport: The Case of Fencing
Most athletes in order to achieve satisfactory results have to train long and hard. However, some of them, aiming at victory at all costs, resort to doping. The benefits of winning are relatively small when compared to the damage of the human organism caused by doping. The phenomenon has been present in cycling, athletics, boxing, football, weightlifting and many other sport disciplines for many years. In fencing only few instances of the use of illegal substances during the competition were revealed, which was reflected in Polish and foreign sports press. In many European countries the use of doping is regulated by the anti-doping rules, regulations and policies that define these practices as penal acts. In Poland the problem of doping was first regulated by the Act on Physical Culture of 1984 and 1996 as well as the he Act of 1985 on Prevention of Drug Abuse. The regulations of FIE fencing competitions, prepared on the request of Polish Fencing Federation on the basis of the FIE competition rules in the disciplinary regulations of competitions prohibited the use of artificial stimulants during or before the competition. The aim of this paper is to present the threats in modern sports with particular attention to the phenomenon of doping in fencing, as well as in social terms. It is a case study and a study of selected issues connected to doping in contemporary sports. The author poses the question whether the fencers often are tempted to support their bodies with doping in order to achieve better results than the representatives of other sports? What percentage of the fencers were using illegal stimulants? Is there is a problem of doping in fencing, and if so, what is its scale
Lymphoedema — assessment of the diagnostic and prognostic value of upper arm dynamic lymphoscintigraphy in patients after mastectomy
Obrzęk limfatyczny po operacyjnym leczeniu raka piersi z usunięciem węzłów chłonnych pachowych często
utrudnia powrót do prawidłowej aktywności z powodu zwiększenia objętości kończyny, zniekształcenia,
ograniczenia ruchomości w stawach i przykurczów mięśniowych. Celem pracy była ocena korelacji między
dynamiką przepływu chłonki w obrębie kończyn górnych u pacjentek po leczeniu operacyjnym raka piersi
a skutecznością leczenia obrzęku limfatycznego. Badaniami objęto 20 kobiet. Badania wykonano za pomocą
g-kamery Varicam, firmy ELSCINT po podskórnym podaniu 0,5 ml Nanocollu znakowanego 1 mCi 99mTc.
Stopień niewydolności układu chłonnego określano na podstawie 4-stopniowej skali Weissledera, z wykreśleniem
krzywej narastania aktywności w badanej grupie węzłów chłonnych. Na podstawie wykonanych
badań stwierdzono, że większy stopień redukcji obrzęku mierzony procentowym ubytkiem objętości wyjściowej
korelował z mniejszym stopniem niewydolności chłonnej określonej na podstawie limfoscyntygrafii
dynamicznej (p < 0,05). Limfoscyntygrafia dynamiczna w połączeniu ze statyczną pozwala więc ustalić nie
tylko stopień niewydolności układu chłonnego, ale również określić rodzaj i przewidywaną skuteczność
terapii.Upper arm lymphoedema is a very debilitating consequence of breast cancer therapy because of excessive
limb volume, shape distortion, mobility limitation. The aim of the research was to assess the correlation
between lymph outflow impairment and complex decongestive therapy (CDT) effectiveness in breast cancer
related lymphoedema. There were 20 women included into the procedure. Lymphoscintigraphy (LAS) was
made on g-camera Varicam (ELSCINT) after subcutaneous injection of 0.5 ml Nanocoll labeled 99mTc (1 mCi).
Lymphatic insufficiency was defined on the basis of four-step Weissleder scale. The results showed that the
higher the level of lymphatic insufficiency the worse effects were achieved during CDT (p < 0.05). Dynamic
LAS combined with the static one have a good predictive value of CDT effects
The potential influence of the applied treatment on selected endocrine and non endocrine parameters and general status of the patients with cancer cachexia syndrome
The aim of the study was to establish correlation between the grade of cachexia and possible endocrine and
non endocrine factors and the type of therapy (NSAID, corticosteroids, progestagens). 98 patients aged 36-70 years (mean - 55 years) with various degree of cancer cachexia were qualified to the study. Statistically
significant, positive correlation between weight loss and: anorexia (p < 0.01), pain intensity (p < 0.01),
grade of depression (p < 0.01), higher values of cortisol (p < 0.05) was detected. The progestagens are the
most effective agents in the treatment of cancer cachexia, but good symptom control and psychological

Zespół wyniszczenia nowotworowego. Wpływ leczenia na wybrane parametry kliniczne i biochemiczne. Doniesienie wstępne
Celem badań było ustalenie korelacji między stopniem wyniszczenia nowotworowego a stwierdzanymi
zaburzeniami endokrynnymi oraz rodzajem prowadzonego leczenia. W oznaczeniach hormonalnych u większości
chorych wykazano wysokie stężenia kortyzolu z zaburzeniem dobowego rytmu wydzielania, cechy
zaburzenia konwersji tyroksyny do trójjodotyroniny (zespół niskiej T3) z podwyższonymi stężeniami reverseT3,
niskie stężenia insuliny na czczo i obniżone wartości insulinopodobnego czynnika wzrostu (IGF-1).
W badanej grupie stwierdzono znamienną statystycznie dodatnią korelację pomiędzy zmniejszeniem masy
ciała a brakiem apetytu (p < 0,01), gorszą kontrolą bólu (p < 0,01), stopniem depresji (p < 0,01), wyższymi
stężeniami kortyzolu (p < 0,05). Zmniejszenie masy ciała nie wiązało się znamiennie statystycznie ze stężeniami
IL-6 i czynnika martwicy guza α (TNFα). Uzyskane wyniki wskazują, że wyniszczenie nowotworowe
jest wypadkową wielu czynników. Przewlekła hiperkortyzolemia sprzyja reakcjom depresyjnym, katabolizmowi
białek (współdziałanie z TNFα, głównie w redukcji masy mięśniowej), lipolizie, zwiększa wątrobowe
wytwarzanie glukozy oraz glukoneogenezę. Współwystępowanie wysokich stężeń cytokin oraz hiperkortyzolemii
wywołuje insulinooporność, co sprzyja hiperglikemii poposiłkowej, która szybko hamuje łaknienie
po spożyciu niewielkiej ilości pokarmu. Najlepsze efekty w leczeniu wyniszczenia nowotworowego przynosi
stosowanie progestagenów, ale równie ważna jest dobra kontrola objawów i stan psychiczny pacjenta.The aim of the study was to establish the correlation between the grade of cachexia and possible endocrine
factors and the type of therapy. Disturbances of thyroxin conversion to triiodothyronine - low T3 syndrome,
high revers-T3 values, low insulin and IGF-1 levels. The statistically significant, positive correlation
between weight loss and anorexia (p < 0.01), pain intensity (p < 0.01), grade of depression
(p < 0.01), higher values of cortisol (p < 0.05) was detected. In the examined group, in spite of tendency,
correlation between the grade of weight loss and IL-6 or TNFα values was not observed. The obtained results
indicate, that cancer cachexia is the result of multiple factors. Chronic, higher than normal, cortisol level
induces the depression, protein degradation (together with TNFα, especially in skeletal muscles mass),
lipolysis, higher liver glucose production and gluconeogenesis; together with cytokines leads to insulin resistance,
tendency to postprandial hyperglycaemia, which inhibits appetite after small meal administration.

Disturbances of serum glucose level regulation in patients with advanced neoplastic disease
Zaburzenia gospodarki węglowodanowej oraz częste wahania glikemii u osób z zaawansowaną chorobą
nowotworową stanowią istotny problem kliniczny. Chorych objętych opieką długoterminową, u których
stwierdza się zaburzenia regulacji glikemii, można podzielić na dwie grupy: pierwszą stanowią pacjenci
z jawną cukrzycą, drugą pacjenci z okresowymi zaburzeniami przemiany węglowodanowej niezwiązanymi
z cukrzycą. Czynnikami determinującymi zaburzenia gospodarki węglowodanowej u osób z zaawansowaną
chorobą nowotworową są: przewlekła hiperkortyzolemia, przewlekła hiperkatecholaminemia oraz względna
przewaga glukagonu nad insuliną, efektem czego jest przyspieszenie lipolizy i glikogenolizy. Wszystkie
powyższe mechanizmy prowadzą także niezależnie do znacznego przyspieszenia glukoneogenezy w wątrobie,
z towarzyszącą insulinoopornością oraz zaburzeniami sekrecji insuliny. Do innych czynników zaburzających
prawidłową regulację glikemii zalicza się wydzielane w zwiększonych ilościach cytokiny: TNF-a, IL-1 oraz
interferony. Znajomość podstawowych zasad rozpoznawania i leczenia zaburzeń regulacji glikemii, oparta na
zrozumieniu prowadzących do nich mechanizmów, powinna przyczynić się do lepszej opieki nad tą grupą
chorych, a tym samym poprawić komfort ich życia.Disturbances of carbohydrate metabolism and frequent variations of serum glucose level in patients with
advanced neoplastic disease are the critical clinical problem. Patients, admitted to the long term medical care
units, who have glucose metabolism abnormalities can be divided into two groups: first — are the patients
with symptomatic diabetes mellitus, and second — are the patients with periodic carbohydrate metabolism
abnormalities without clinical manifestation of diabetes mellitus. These glucose metabolism abnormalities in
patients with malignancy can be determined by such factors as: chronic excess of cortisol and katecholamines,
and relative predominance of glucagons over insulin, that causes acceleration of lypolysis and glycogenolysis.
Mechanism mentioned above lead independently also to marked increase in liver guconeogenesis in association
with insulin resistance and disturbances in insuline secretion. Other factors, that may affect serum
glucose level regulation, are the cytokines released in excess like TNF-a, IL-1 and interferon.
Knowledge about diagnosis and treatment principles of glucose metabolism derangements based on understanding
of mechanism that can lead to these disorders, should contribute to better care of this group of
patients, thereby improve quality of their life
Preliminary experience with the use of methadone in the treatment of cancer pain
Background. In an open clinical study analgesia and side effects of methadone
were assessed and the calculation of equianalgesic doses of oral morphine and
methadone used in our patients was appraised.
Material and methods. Methadone was administered in 12 opioid - tolerant
patients with severe cancer pain. Patients received methadone because of inadequate
pain control (NRS > 5) (number of patients in brackets) during treatment with
morphine (4), transdermal fentanyl (3), morphine, ketamine and transdermal fentanyl
(1), tramadol (1), unsatisfactory analgesia with severe drowsiness during morphine
with ketamine treatment (2) and very strong pain with nausea during morphine treatment
(1). We used the following dose ratios while converting oral morphine equivalent
to oral methadone: 4:1 (up to 100 mg morphine daily), 6:1 (100–300 mg morphine
daily) and 12:1 (daily dose of morphine over 300 mg) although in this study because
of daily doses of oral morphine over 100 mg we applied only the last two dose
ratios. The starting daily dose of oral methadone in patient treated with tramadol
was 9 mg. The single dose of oral methadone did not exceed 30 mg regardless the
daily dose of oral morphine before the switch to methadone. After starting methadone
we withdrew previous opioid treatment completely in 10 patients and 2 patients
were treated concomitantly with methadone and other opioids. The mean equivalent
daily dose of oral morphine before switching to methadone was 750.8 ± 464 mg.
Methadone was administered usually 3 times a day and 11 patients were treated
with oral methadone in water solution, 1 patient received the drug rectally in
suppositories.
Results. The mean time of the methadone treatment was 31.7 ± 21.2 (range
7–82) days and the starting doses were increased in 11 of 12 treated patients.
The mean daily doses varied from 46 ± 19.9 mg at the beginning till maximal 135.3
± 99.9 mg and 117.7 ± 97.5 mg at the end of methadone treatment. Good analgesia
expressed by decrease in pain intensity to NRS < 3 was achieved in 6 patients,
partial effect (NRS 3–5) was present in 5 patients, unsatisfactory analgesia was
observed in 1 patient (NRS > 5) who ceased methadone after 7 days of the treatment.
The most frequent side effects were drowsiness (5 patients), constipation (5 patients),
nausea and vomiting (2 patients). We did not observe serious adverse reactions
especially respiratory depression, which would cause cessation of the treatment.
Conclusions. The results of this preliminary study confirmed high analgesic
efficacy, good adverse event profile of methadone, safety and the effectiveness
of the applied method of morphine to methadone dose calculation for the oral route.Wstęp. W otwartym badaniu klinicznym dokonano oceny analgezji i objawów
ubocznych metadonu oraz zastosowanego sposobu przeliczania dawek równoważnych
morfiny i metadonu, podawanych drogą doustną.
Materiał i metody. Metadon podawano 12 chorym z bólem nowotworowym o silnym
natężeniu, wcześniej leczonych innymi opioidami. Wskazaniem do stosowania metadonu
była niedostateczna kontrola bólu (NRS > 5) (w nawiasach podano liczbę chorych)
podczas leczenia morfiną (4), przezskórnym fentanylem (TF) (3), morfiną, ketaminą
i przezskórnym fentanylem (1), tramadolem (1), nasilone bóle i senność podczas
terapii morfiną i ketaminą (2) oraz bardzo silne bóle i nudności podczas leczenia
morfiną (1). Przyjęto następujący sposób przeliczania dawek równoważnych opioidów:
przy ekwiwalentnej dawce dobowej doustnej morfiny do 100 mg dawkę dobową doustnego
metadonu obliczano według przelicznika 4:1, przy dawce dobowej doustnej morfiny
100–300 mg według przelicznika 6:1, przy dawce dobowej doustnej morfiny powyżej
300 mg według przelicznika 12:1, przy czym z powodu ekwiwalentnych dawek dobowych
doustnej morfiny powyżej 100 mg w badaniu zastosowano tylko ostatnie dwa przeliczniki.
U chorej leczonej tramadolem początkowa dawka dobowa doustnego metadonu wynosiła
9 mg. Niezależnie od wielkości dawki dobowej doustnej morfiny początkowa jednorazowa
dawka metadonu nie przekraczała 30 mg. Po rozpoczęciu leczenia metadonem u 10
chorych całkowicie odstawiano wcześniej przyjmowany opioid, 2 chorych otrzymywało
równocześnie metadon i inne opioidy. Średnia ekwiwalentna dawka dobowa doustnej
morfiny, przed zamianą na metadon, wynosiła 750,8 ± 464 mg. Metadon podawano najczęściej
3 razy dziennie - 11 chorych otrzymywało lek doustnie w roztworze wodnym, 1 chorego
leczono metadonem podawanym drogą doodbytniczą w postaci czopków.
Wyniki. Średni czas leczenia metadonem wynosił 31,7 ± 21,2 dni (zakres
7–82), średnie dawki dobowe leku zwiększyły się u 11 spośród 12 leczonych chorych
i wynosiły na początku 46 ± 19,9 mg, maksymalnie 135,3 ± 99,9 mg, na końcu terapii
117,7 ± 97,5 mg. Korzystne efekty przeciwbólowe, wyrażające się zmniejszeniem
natężenia bólu do NRS < 3, odnotowano u 6 leczonych chorych, efekt częściowy (NRS
3–5) stwierdzono u 5 chorych, u 1 pacjentki efekt analgetyczny oceniono jako niedostateczny
(NRS > 5) i po 7 dniach zakończono leczenie. Spośród objawów ubocznych najczęściej
występowały senność (5 chorych), zaparcie stolca (5 chorych) oraz nudności i wymioty
(2 chorych). Nie obserwowano poważnych działań niepożądanych, zwłaszcza depresji
oddechowej, które spowodowałyby konieczność przerwania leczenia.
Wnioski. Wyniki wstępnego badania potwierdziły wysoką skuteczność analgetyczną
i dobrą tolerancję metadonu oraz bezpieczeństwo i przydatność zastosowanego sposobu
przeliczania dawek morfiny na dawki metadonu, podawanych drogą doustną
